I can't remember if insulin shots hurt non-diabetics or not. But if he is diabetic, why wouldn't he be in good shape? It's not a death sentence or anything.
As long as you take care of yourself you can survive just fine with diabetes. If you don't take care of yourself, well you won't do quite so well.
